A service for people living with a diagnosis of dementia or cognitive impairment and their carers.

Care Partnerships Australia is a private fee for service consultancy that will provide the customer with:

  • information about how to live well with dementia and care planning,

  • an in-home consultation that includes an assessment and care plan,

  • a communication guide that helps you to speak to and understand your loved one when communication is difficult because of a diagnosis of dementia.


Our goal is to maximise the human rights and choices for people living with dementia and deliver our services with compassion and integrity.

Michelle Harris-Allsop

I am a dementia coach for service providers in Australia and Singapore. I support and mentor staff in building care partnerships with people who live with a diagnosis of dementia.


I build the capability of teams working in private hospitals and residential care services with people who have a diagnosis of dementia.


As a coach I provide practical knowledge to improve the standard of customer service and lived experience for the person who has dementia. My goal is to enable choice and autonomy for all clients.


This involves a mentorship partnership with the service provider, looking at building knowledge about

  • how to improve communication and assessment with a client so they are involved in a care partnership

  • how to maximise well-being for a person living with dementia and their family

  • building support in the environment via signage and assistive technologies

  • providing support and coaching with advance care planning

  • providing education to the carers so they can understand the lived experience of the person living with a diagnosis of dementia

  • building care partnerships with the clients I support to improve standards of service.


Michelle is a Registered Nurse- Dementia Consultant at Care Partnerships Australia in Western Australia. She worked within the aged and community care sector since 2003, and her achievements promoting culture change for people living with dementia were recognised as an award finalist in the Western Australian Nursing and Midwifery Awards (Aged Care).


Through working with Michelle over the past ten years I have observed Michelle’s commitment and passion to a person-centred approach for the health and well-being of people living with dementia, and achieving this in partnership with people living with dementia, families and aged care staff.  Michelle’s expertise and ability to educate others about enhancing the dignity and quality of life of people living with dementia is unique.


I strongly recommend Michelle as a Dementia Consultant for people living with dementia and their families to provide practical problem-solving assistance and support, and aged care providers who require evidence based practical training and consultancy services for staff.
